No. In order to accept insurance,medicare & medicaid, there is credentialing required from these companies that would not allow us to practice functional medicine and hormone therapy to its fullest potential. Additionally these companies require an abundance of paper work and time commitments to submit claims. We are able to keep our prices low by cutting out the middle man! Give us a try and you won't be disappointed. 

We accept cash, check, credit card and HSA cards.

Yes. In order to keep our prices low, we will charge a fee for credit card/HSA card processing. Please reach out if you would like to learn more about the fee. You can avoid this fee by paying with cash or check!

Yes we do! Seeing a provider from the comfort of your home is possible with Choice Health.  We know it can be difficult to make it in the office when you are housebound or a family member is not feeling well. To cover the extra time & mileage spent on the road, we will have a $30 fee if you are within 10 miles of Rock Valley, and all other areas up to 23 miles will be a $60 fee. Presently, we are only able to make house calls in the state of Iowa.

Yes, we are licensed to prescribe medication in the State of Iowa. 

Yes, depending on your conditions we may have alternative treatment plans other than a prescription to help you. Check out our supplements page as well!

Currently we do not offer vaccines. We can make recommendations for other facilities that offer vaccinations if needed.

Yes, your information is safe and meets HIPPA requirements. We use Athena Health as our electronic health records (EHR) and they have close to 150,000 providers/physicians that use their system . Your privacy and information security is our top concern.

We are able to treat you from the functional medicine side as well as any issues you may have relating to primary care.  We will be a great fit for you as your primary care provider and would be honored to offer you a continuum of care . We understand some medical scenarios may require you to seek care from a primary care provider who accepts insurance, and are more than happy to help you navigate this as well.

In order to best serve you and operate in our scope, we generally will need to schedule an appointment with you before we offer any medical advice. While we love to talk with each and every one of our potential and current patients, unfortunately we would be on the phone all day and we would be setting ourselves up for liabilities without making a full physical assessment. We understand that many medical issues can come up that need professional advice or guidance and will work with you to get an appointment as soon as possible (often the same day).

We understand that there is a vast array of labs and tests that are available online and may be offered at a lower price. We will draw the labs for you and spin them on our centrifuge for a fee should you chose to use this option. However this fee does not include interpretation.  We will offer to interpret them, but this fee will be substantially more than what we would charge if you would use the recommended labs that we currently offer. You can think of it this way, imagine living in your house for the last 10 years and then you moved to a different house the next day. You would bring all the same things into this new house, but you would have to figure out where everything is located. This is the same with different testing companies. They may offer the same labs, but we would have to spend the time trying to figure out where each of the lab values were located and organize them in a way we can efficiently interpret them.
